Exactly two years after a plane crash claimed the lives of a Penn State lacrosse player's family, the National Transportation Safety Board released a report stating that pilot error was the probable cause of the crash.
The plane's pilot was Jeffrey Jacober, 51, father of former men's lacrosse team captain Michael Jacober.
Jeffrey Jacober failed to maintain sufficient airspeed to avoid stalling during the landing approach, according to the report.
The crash occurred at about 1:45 p.m. March 26, 2005, near the Centre County Correctional Facility. The accident site was about three miles from the target runway at University Park Airport.
"Factors associated with the accident are the inadvertent stall/spin, the pilot's failure to follow procedures/directives, and clouds," the report stated.
The pilot also failed to properly configure the airplane's navigation system to allow the autopilot to lock on the University Park Airport's instrument landing system signal, an analysis concluded.
No anomalies were found in the airplane, its engine or its flight navigation systems, according to the report.
"Witnesses reported seeing [the plane] spinning in a nose down, near vertical attitude before it collided with the ground," according to the report.
The temperature was 39 degrees, and the sky was overcast at the time of crash, the report indicates.
Pilots in the area when the crash occurred said icy conditions existed in the clouds near the airport.
However, first responders to the accident site said no ice was on the airplane, according to the report.
Six people were killed in the crash, including Jeffrey Jacober, his wife, Karen, 49, and their son Eric, 15. Jacober family friends -- Gregg Weingeroff, 49; his wife, Dawn, 42; and their son, Leland, 10 -- also died in the crash.
The flight began in Naples, Fla. Both families were traveling to State College to watch one of Michael Jacober's lacrosse games.
Michael Jacober did not respond to an e-mail request for comment.
