Penn State students residing on campus will now have to complete a Meningococcal Vaccination Certification/Exception Form and file it at the University Park Assignment Office in Shields Building or with the appropriate Housing and Food Services office at Commonwealth Campuses.
By Aug. 17, students must show proof of vaccination or fill out numerous forms indicating why they should be exempt. Students who do not have this information on file will not be eligible to participate in the 2003 Spring Semester academic registration, which begins the first week of September.
A letter and the vaccination clinic request form have been mailed to each student who has not filed the papers. Students may receive the vaccination from their own health care providers and mail the appropriate forms by Aug. 17. Also, the university is hosting vaccination clinics at most Penn State campuses prior to the Fall Semester. To participate in the clinics, a student must return the request form by Aug. 17.
A vaccination clinic will take place from 10 a.m. to 5 p.m. on Aug. 24 in Ritenour Building. Vaccinations are $85 and payment may be made by check, cash or credit card, or charged to a student account.
